Depending on your computer’s operating system and software, you may:

1. Print out the form, complete it by hand, and mail it to Eat by Numbers, 300 Cedar Ave., STE3, Allenhurst, NJ 07711.

OR

2. Type directly onto the Food Diary or Personal Information Form in your browser window, then print it out and mail it.

OR

3. If you are able, ( i.e. have a PC and Adobe Standard or Pro), type directly onto the Food Dairy or Personal Information Form in your browser window, save the document to you hard drive, and then send the document as an attachment to mary@eatbynumbers.com. Your email is kept confidential and secure by ZixCorp eSecure encription services.
